    Appendix 1

    Appendix 1 – Certifying Documents for Submission

    You are required to submit certified copy of these documents when accepting your offer. These documents are to be certified by at least one of these:

    • Registrar of your previous College or University
    • School Principal or Vice Principal of your previous high school
    • Notary Public
    • Commissioner of Oath
    • IMU Admissions personnel

    Ensure that the required documents are certified correctly as IMU reserves the right to refuse certified documents on the grounds of incorrect certification.

    Appendix 3 – Type of Blood Screening Required

    Student enrolling under the following programme enrolling into IMU are required to under these screening:

    Immunization is only required if the Hepatitis B (Antigen) reading is either below 10 mIU/ml, Negative or Non-Reactive.

    An immunization card or a letter from the doctor is required to prove that the student has completed at least the first dose of the Hepatitis B immunization.

    Appendix 4 - Sample of Passport-sized Photos for Student ID Card

    Format required:

    • Forehead and nose (t-zone area) must be clear from any obstruction
