- Health Review and Medication Adherence Review: the elderlies had blood glucose level and blood pressure were monitored and recorded
- Caregivers and person-in-charge were counselled on medication safety – pill box use, medicine use, storage, labelling, administration and disposal of the medications
- Medication storage were reorganized and rearranged for easy accessibility of the caregivers
Outcome
- More than 90% of the elderlies were screened and reviewed on their health and medication
- More than 50% of the elderlies utilized pill boxes that were provided
- More than 50% improvement were observed in medication adherence among the elderlies, and in caregivers and person-in-charge’s over-the-counter product knowledge in storage and use
- Students were able to establish a better sense of humanity and dignity, alongside with inculcating a sense of responsibility, understanding, patience and empathy towards the elderlies
